SOURCE, or Service Options Using Resources in Community Environments, is a state program designed to provide elderly disabled and low-income patients with a wide range of long-term health services, either in their home or community. Often, the program can help avoid nursing home care and unnecessary hospital stays. UniHealthSMSolutions is a participating SOURCE site with two locations serving Metro Atlanta and North Georgia. Counties served include Lumpkin, Fannin, Union, Gilmer, Cobb, Gwinnett, Fayette, Henry, Clayton, Rockdale, Douglas, Murray, Pickens, Cherokee, Walker and Towns.

SOURCE can help elderly and disabled Georgians on Medicaid under SSI stay healthy and stay at home while receiving medical services at little or no expense to patients.

In-Home services offered include: Home health, Personal support, Homemaking, Home-delivered meals, Emergency response, Caregiver respite, Transportation, and Errands.
