Q: What is UniHealthSM Solutions?
A: UniHealthSM Solutions is an independent care management company that provides a single point of access to the array of services associated with the UHS-Pruitt family of providers. Services are offered throughout the state of Georgia and parts of the Carolinas. Once making a referral through the UniHealthSM central intake line, Care Managers are assigned to coordinate and manage the care delivered through affiliated organizations such as United Hospice, United Homecare, or any one of our numerous skilled nursing facilities.
Q: How do I make a referral?
A: The process is simple and convenient. Simply call Central Intake at 770-925-1143 or toll-free 866-864-4325 and indicate that you have a referral. From this point, we handle everything for you. Depending on the type of care you are seeking, whether it is Home Health, DME, SNF services, Hospice, you will be forwarded to an Intake Coordinator. From there, the Intake Coordinator will gather all pertinent information in order to verify benefits, assign the case to the appropriate care manager, and ultimately arrange for the delivery of care to the patient. It's that easy!
Q: What is Hospice?
A: Hospice care is a service provided to people who are terminally ill and have six months or less to live. It focuses on palliative (comfort) care which seeks to relieve pain associated with illness without actually curing it. Hospice treats not only the body, but also the mind and spirit. UniHealth'sSM sister company, United Hospice, provides this care.
Q: What is Home Health Care?
A: United Homecare, UniHealth'sSM sister company, offers a full range of Medicare certified home health services in the home or assisted living facility. These services include skilled nursing, home health aids, and physical, occupational and speech therapy. By coordinating with the physician and family, our staff is able to develop an individual plan of care. Services are designated with one goal; to assist patients in reaching their optimum level of function while in the comfort of their residence.
